Payments, Insurance & Billing

Please provide our office with your current insurance information. If you don’t have insurance, please discuss alternative forms of payment with our office prior to your service.

Copayments, Coinsurance
& Deductible

All applicable copayments, coinsurance, and deductibles are due at the time of service. We accept Visa, Mastercard, Discover, American Express, Cash, and personal checks with Valid IDs.

Insurance Authorizations & Referrals

If your insurance requires a referral or authorization to be seen, please obtain that prior to your appointment.

Patient Accounts/Billing

If you have questions regarding future appointments, please call our office at 904-208-2550. Please contact 904-472-2300 if you have billing questions. You can pay any patient balances in our office or online.

We try to obtain your insurance benefits 12-24 hours in advance from your appointment. If you have any questions about your responsibility please contact your insurance company or our office before your appointment.

Our staff are available by phone to triage your medical questions Monday through Thursday from 8:00 a.m. to 4:30 p.m. and Friday from 8:00 to 4:00pm. Calls are returned throughout the day in order of priority. If the nurse or staff member is unable to answer your questions, they will consult the physician/provider and return your call as soon as possible. For prescription refills, please call your pharmacy.

We have a well trained team of nurses and physicians on staff 24 hours a day for all of your emergency needs.
