Day of Your Appointment

Please bring your identification and insurance information on the day of your appointment and be prepared to pay any co-pay or deductible applicable to your insurance plan. Our staff is happy to assist you before your appointment in determining what your payment may be.

Upon arrival, a front office coordinator will provide you with any forms needed to get you checked in quickly. A back office coordinator will call for you as soon as possible and will also inform you if any wait time exceeds 20 minutes. If you’ve been waiting for 20 minutes please let your front office coordinator know. Your time is valued, and we want to do everything possible to limit wait times. If you require a new appointment time we are happy to reschedule it for you.

Arrival Time

We ask all new patients to arrive 30 minutes prior to their appointment time so that staff may enter all necessary information into your medical record. Established patients should arrive 15 minutes prior to their appointment.

Rescheduling an
Appointment if Late

At North Florida of Jacksonville, we make every effort to keep your appointment on schedule. If you are running 15 minutes past your original appointment time we may need to reschedule you to ensure that other patients do not miss their appointments.

Cancellation Policy

Please give us a call 24 hours prior to your appointment if a cancellation or reschedule is needed to avoid a late cancellation charge. This will allow us to accommodate other patients who need to be scheduled.
